animal report form for notebooking??? ami*??

i am sure someone linked to a wonderful report form for notebooking, i thought it was at homeschoolshare, but i have searched thoroughly and can not find it! :(

does anyone remember this? it had a place to put the animal picture and information about habitats, etc...
